You can get in contact with us if you believe you need support.
We're also happy to be contacted by someone acting on your behalf, but they will need your consent first.

First, you should make sure you're contacting the correct local authority.
This is usually decided by where you live.
You can check which council area you live in. If it's in the London Borough of Hounslow, it's us you need to contact.
How to contact us
You first need to complete a brief form. We call this an assessment form.
The assessment form gives us a headstart in understanding your circumstances and thinking about how we might best be able to help.
We'll then get in contact with you to either:
- provide helpful guidance and signposting to useful services
- arrange a more thorough needs assessment
