Decision to transfer to adult social care
Our decision to transfer you to adult social care, following the transition assessment, is based on if you:
- have needs for adult care and support
- have such needs and the available support can meet some or all of them
- have such needs but decide the available support is not going to meet them (either because the needs are not eligible or because the needs are already being met)
After this assessment, you will either:
- need to be transferred to one of the two adult social care teams:
- not need to be transferred to adult social care services

Adult learning disability and autism service (ALDAS)
ALDAS provides social care support for young people from the age of 17, who have a learning disability, autism or both. ALDAS is split into two teams:
- Independent futures team: Works with young people aged 17 to 24 years with the aim of increasing their independence
- Community team: Supports people aged 25 and over to improve their social and interpersonal skills and wellbeing

Adult localities mental health team (ALMHT)
ALMHT is made up of mental health social care professionals who aim to increase the wellbeing of Hounslow residents and support the recovery of those with a long-term mental health illness.
