This service empowers adults with disabilities (particularly learning disability and autism) to overcome barriers and thrive.

It does this by supporting individuals to find and access activities, opportunities and services to reduce isolation and move forward in their lives.

What we offer

We offer person-centred advice, signposting, and support to manage change and prevent isolation by connecting the resident to statutory, specialist and community partners to reach their goals.

This can range from brief advice and introductions to more in-depth keywork sessions to achieve specific outcomes agreed with the resident. It can also include working with the resident’s family where appropriate.

Examples of support we can provide:

  • Support with accessible information
  • Signposting and referral to suitable support agencies
  • Help to access community groups and prevent isolation
  • Support to manage life changes in circumstances or significant life events such as changes at home, disability or moving between services
  • Help to explore new opportunities to stay active in your community or build skills for daily living or pre-employment

Eligibility

To be eligible to use this service you need to be:

  • aged 18 or over
  • living in the London Borough of Hounslow
  • have a disability as defined by the Equality Act 2010 (GOV.UK)
  • at risk of becoming isolated or losing contact with services
  • ready to engage with support and signposting with other professionals and services

The service is not intended to replace ongoing floating support or personal assistance needs. For some areas of need it may be necessary to seek specialised professional support.

We often work in partnership with adult social care to respond to safeguarding and wellbeing concerns. The level of involvement will vary according to individual circumstances and existing support.
