Getting help

As well as things you can do yourself to prevent falls, there are other ways we might be able to help.

Home assessment

Our home safety team (HST) can visit your home to Identify risks and hazards which may cause a slip, trip or fall.

The HST will look at:

  • your environment, assessing any trip hazards, adaptation or equipment that might help
  • your health and wellbeing needs, such as:
    • vision
    • dizziness
    • memory
    • bone health
    • feet
    • bladder/bowel problems
    • nutrition
    • hydration
    • mental health
  • medicine management, health conditions, access to a GP and social prescribing service
  • movement and exercise, staying active, OTAGO classes
  • fear of falling and steps to take if you do fall
  • isolation and befriending services
  • providing information about services such as financial support, handy person, transport options, telecare and equipment for assistance

The Hounslow falls prevention and bone health service aims to help adults to: 

  • reduce the rate and risk of falls and fragility fractures
  • improve their health and mobility and sustain their independence
