Transitions with an EHCP

Sometimes, a transition may mean you change the place where your child studies. This could mean going from an infant school in one building, to a junior school in another, or a secondary school in a completely different place. These changes are known as phase transfers. 

There are two phase transfer deadlines in the year. We must have issued a final EHCP naming the education setting your child will be attending in the next academic year by these deadlines.  

The deadlines are different depending on whether your child is transitioning to school (primary or secondary) or post-16 education.  

The deadline for school age phase transfers is February 15.  

The deadline for post-16 phase transfers is March 31.  

You need to contact the Hounslow SEND team and tell them of your preferred school.

They will then find out whether the school you have requested is suitable for your child’s needs through the EHCP consultation process

Year 9 transition

Children and young people with an EHCP have an annual review every year throughout their time at school or college. The annual review checks that the support they receive is still appropriate and effective. 

A year 9 annual review marks the start of planning for your young person’s transition to adulthood
